Quick Answer: Getting ABA claims paid in Texas depends on catching the details before they become denials. Cube Therapy Billing checks authorizations, CPT codes, billed units, provider information, modifiers, and payer requirements before claims go out, then follows unpaid or denied claims through resolution. Texas Medicaid managed care claims generally must reach the correct MCO within 95 days of the date of service, while commercial plans subject to Texas autism coverage requirements can follow a different set of billing and authorization rules.  TRICARE also covers qualifying ABA services through its Autism Care Demonstration. Our job is to keep those payer differences from slowing down your revenue. We track the claim from submission through payment, work denials when something goes wrong, and follow aging A/R until there is a clear resolution.

WHY TEXAS ABA PRACTICES COME TO US

ABA billing in Texas gets complicated fast. Practices juggle Medicaid MCO rules, commercial authorizations, TDLR licensing requirements and credentialing with multiple payers. Small gaps quickly turn into denied claims or aging AR.

Fewer Repeat Denials

We do more than correct denied claims. We look for the reason behind the denial, fix the issue, and help prevent the same problem from showing up again across future claims.

Better Authorization Control

We track approved CPT codes, units, service dates, and authorization limits so your team knows what can be billed before sessions turn into unpaid claims.

Billing That Matches Each Payer

Texas Medicaid MCOs, commercial insurers, and TRICARE do not all follow the same rules. We adjust the billing process based on the payer instead of using one setup for every claim.

Faster Follow-Up on Aging A/R

Older claims need consistent attention. We follow unpaid balances, check claim status, respond to payer requests, and keep working the account until there is a clear outcome.

Correct Provider and Modifier Setup

We check provider credentials, service type, rendering information, and required modifiers before submission to reduce avoidable claim errors.

TEXAS BILLING RULES

Texas ABA Billing Rules Your Team Needs To Get Right

ABA medical billing in Texas depends on Texas Medicaid managed care, commercial plan mandates under the Texas Insurance Code, prior authorization, TDLR provider licensing, CPT-specific rules, and requirements that vary by MCO. A claim can be documented correctly and still be delayed or denied if the authorization, provider, modifier or billed units don't match what that Texas plan requires.

Texas Medicaid ABA Coverage

Under Age 21

Prior Authorization Required

Medical Necessity Documentation Required

Eligibility: Texas Medicaid covers ABA for members under 21 with an autism diagnosis through Texas Health Steps Comprehensive Care Program (CCP). Prior authorization is required, and treatment is reauthorized on a plan-specific schedule.

Coverage details: ABA services must be medically necessary and supported by a comprehensive diagnostic evaluation and an individualized treatment plan from a licensed behavior analyst.

Managed care: STAR, STAR Kids and STAR Health MCOs manage this benefit, and each plan sets its own authorization workflow, documentation rules and renewal timelines.

Units and codes: ABA CPT codes are billed in 15-minute units. The covered code set includes 97151, 97152, 97153, 97154, 97155, 97156, 97157, 97158, 0362T and 0373T.

Timely filing: Texas Medicaid claims must be filed within 95 days of the date of service. Late claims are denied outright, so authorization and billing have to stay in sync.

Prior Authorization

ABA prior authorization requirements: Texas Medicaid ABA billing requires prior authorization for assessments and treatment. Initial requests must include an LBA-signed ABA treatment plan, medical necessity documentation, and prescribing provider authorization. Treatment approvals generally begin with 90 days, with extensions required for continued care.

ABA CPT codes and billing modifiers: Texas Medicaid covers CPT codes 97151, 97153, 97154, 97155, 97156, and 97158 when coverage requirements are met. CPT 97151 requires modifier HO; codes 97155, 97156, and 97158 use HO or HN based on provider qualifications. Codes 97153 and 97154 may use HM for behavior technicians.

ABA authorization and CPT unit limits: CPT code 97151 allows up to 24 units (six hours) for ABA assessment or reassessment. ABA CPT codes 97153–97156 and 97158 require approved units. Practices must track Texas Medicaid authorization dates, remaining units, and renewals to prevent claim denials.

Concurrent ABA billing restrictions: Texas Medicaid generally prohibits overlapping ABA therapy billing for the same child, with limited exceptions for separate caregiver services. ABA insurance claims must reflect actual treatment time and exclude routine administrative work.

ABA telehealth billing rules: Texas Medicaid permits qualifying audio-video ABA telehealth services under CPT 97151, 97155, 97156, and 97158 with modifier 95. Assistant behavior analysts and behavior technicians cannot provide these services remotely under this policy.

How Our Multi-Payer ABA Billing Services Handle Different Payers

Our multi-payer ABA billing services are built to reduce avoidable denials, prevent authorization-related billing errors, and keep claims moving across different insurance plans.

At Cube Therapy Billing, we do not use one billing process for every payer. We adjust claim setup, authorization checks, modifiers, provider details, and follow-up based on the specific rules of each commercial payer, Medicaid program, and managed care plan.

New Jersey's autism mandate requires state-regulated plans to cover medically necessary ABA. It took effect in February 2010 and is one of the stronger mandates in the country.

The original under-21 age cap and the 30-visit limits on OT, PT, and speech came off through later amendments, so adults with autism can now access covered ABA under state-regulated plans.

We check payer-specific billing requirements before claims go out, which cuts avoidable rejections and denials. When a payer denies or delays a claim, our team reviews the reason, corrects the issue and follows up using that payer's process.

We update billing workflows whenever commercial plans, Texas Medicaid or the MCOs change their requirements.

Payer policies change often. Cube therapy billing verifies the current version of each payer's policy, fee schedule, and authorization requirements at intake and monitors updates for every client. The summary above reflects publicly posted payer policy at the time of writing and is not a guarantee of coverage.

Why choose Cube

Why Texas Practices Pick Cube Therapy Billing

Plenty of ABA billing companies can submit a claim. Far fewer understand how provider type, authorizations, modifiers, MCO rules and Texas's mix of Medicaid, commercial and TRICARE payers each affect reimbursement.

ABA-specialized, not general medical billing

Our team lives in 97151 to 97158, concurrent billing rules, supervision modifiers, and authorization cycles. Your claims aren't learning material for a generalist biller.

Texas Medicaid Billing Awareness

We follow Texas Medicaid's current behavior analysis fee schedule and code-specific billing rules so claims can be reviewed before submission. Florida Health Care Agency

First in the industry

SparkzABA: AI powered RCM workflow management

Every Cube client runs on SparkzABA, our RCM workflow management software with AI powered workflows, the first of its kind in the ABA billing industry. It tracks every claim, authorization, credentialing file, and eligibility check in real time, flags risks before they become denials, and gives you a live window into your revenue instead of a month-old PDF report.

Proactive denial prevention

We catch problems before claims go out: missing auths, wrong modifiers, unit overages against Horizon's daily limits. That's how our denial rate stays under 3%.

Credentialing and billing under one roof

From Texas Medicaid provider enrollment and ABA credentialing to CAQH updates, prior authorizations, claim submission, denial management, and A/R recovery, we manage your entire ABA revenue cycle across Texas Medicaid and managed care plans so your team can focus on patient care.

Transparent reporting, HIPAA-compliant operations

Signed BAAs, role-based access, 2FA, and reporting that shows real numbers. You always know your clean claim rate, AR days, and what we're chasing this week.

Clear your doubts

Texas ABA Therapy Billing Questions

These are the questions ABA practice owners actually ask us on the first call.

  • ABA medical billing covers the full insurance billing process for applied behavior analysis services. That usually includes eligibility verification, prior authorization tracking, claim preparation, CPT and modifier review, claim submission, payment posting, denial management, AR follow-up, and credentialing support. The goal is to make sure the service delivered, the authorization, the documentation, and the claim all match before the payer processes it. Cube Therapy Billing already structures its ABA billing services around that full revenue cycle.

  • Cube Therapy Billing works with the core ABA CPT code set, including 97151 through 97158, along with 0362T and 0373T where applicable. Claims are reviewed for coding, modifiers, rendering provider details, place of service, authorization, and payer-specific requirements before submission.

  • Yes. Cube manages ABA prior authorizations by tracking approved units, authorization dates, payer requirements, and upcoming expirations to help prevent avoidable claim denials. We also coordinate authorization details for different care settings, including Virtual ABA Therapy in New Jersey, where coverage and approval requirements may vary by health plan.

  • Yes. Cube’s denial management process includes reviewing the denial reason, correcting claim issues, appealing when appropriate, following up with the payer, and identifying the root cause so the same denial is less likely to repeat. The site positions denial work as part of the full billing cycle rather than simple claim resubmission.

  • Yes. Cube works with Medicaid programs, Medicaid managed care plans, and commercial payers. On its current site, Cube specifically references payer experience that includes Medicaid MCOs and commercial insurers such as Aetna, Cigna, UnitedHealthcare, and BlueCross BlueShield.

  • Yes. Aging AR cleanup is usually part of onboarding. We audit unpaid claims, rework what's recoverable within timely filing limits, and give you an honest write-off recommendation on what isn't. Many practices recover meaningful revenue from claims they'd assumed were dead.

  • In most cases, yes. Cube says it works with more than 25 EHR and practice-management systems, including platforms such as CentralReach, AlohaABA, Rethink, Lumary, and Theralytics. The goal is to work inside the practice’s existing setup rather than force a full system migration.

Find out where your ABA practice is leaking revenue

If your team is chasing denials, babysitting authorizations, or waiting 40+ days to get paid, a 30-minute billing review will show you exactly what's fixable. No pitch deck. Just your numbers and straight answers.
